1. Watch Canada AM (on channel 8 or the local cable or satellite channel in your
area) from March 15th, 2004 to March 26th, 2004 to view the contest spot with
the mail-in address appearing. TO ENTER: simply visit the web site located at
www.ctv.ca and follow the link to Canada AM's webpage, click on the contest page
and complete and submit your entry by following the instructions found at the
site. All entries must include your name, address, age, ten digit day and evening
telephone numbers and e-mail address, if applicable. Limit of one entry per person
per email address per household. . Entries received on-line shall be deemed to
be submitted by the authorized account holder of the email address submitted at
the time of entry. "Authorized account holder" is defined as the natural
person who is assigned to an e-mail address by an internet access provider, on-line
service provider, or other organization (e.g. business, educational institution,
etc.) that is responsible for assigning e-mail addresses for the domain associated
with the submitted e-mail address. Illegible entries, bulk drop-offs or mechanical
reproductions or facsimile entries are ineligible. Contest starts at 12:00:01
a.m. Eastern Time ("ET") on Friday March 12th , 2004 2003 and closes on March
26th, 2004 at 11:59:59 a.m. ET (the "Contest Period"). There is One
(1) Grand Prize and Fifty (50) Secondary Prizes to be won from all the eligible
entries received during the Contest Period. Limit of one prize per address. No
purchase necessary. Void where prohibited.
The contest sponsors are: CTV Television Inc. and the Canadian Academy of Recording
Arts and Sciences (hereinafter referred to as the "Contest Sponsors").
To enter and to be eligible to win, a person must be a legal resident of Canada
and be over the age of majority in the province or territory in which they reside.
Employees and their parents, siblings and children, and persons domiciled with
an employee of any of the Contest Sponsors, their agents, parent, affiliated or
related companies, subsidiaries, divisions, prize sponsors, and promotional and
advertising agencies and administrators, are ineligible to enter.
2. GRAND PRIZE: There is one (1) Grand Prize consisting of a trip for two (2)
to Edmonton, Alberta. Trip includes: round trip economy airfare for two (2) from
a major Canadian airport closest to the Grand Prize Winner's place of residence
in Canada. (hereinafter referred to as the "Departure Point"); four
(4) nights accommodation (April 1st, 2nd, 3rd and 4th) (based on one room, double
occupancy) as chosen by the Contest Sponsors, two (2) tickets to join "Breakfast
with Jeff" Canada AM taping on April 2nd , 2004 in Edmonton, Alberta ( at
The Westin Edmonton Hotel), two (2) tickets to the Juno Awards show on April 4th,
2004 (seat selection as determined by the Contest Sponsors in their sole and absolute
discretion and authority) , one (1) copy of the Juno Awards 2004 CD, plus Two
Hundred dollars Canadian ($200.00 CDN.) in spending money. Grand Prize Winner
and his/her travelling companion are solely responsible for all costs not expressly
described herein including, without limitation, applicable taxes, fuel/currency
surcharges, ground transportation, meals, beverages, room service, gratuities,
merchandise, telephone calls, insurance together with any required travel documentation,
and all personal expenses of any kind or nature, together with any applicable
overnight layover. Flight dates and hotel accommodation are subject to change
without notice or compensation. Grand Prize Winner and his/her travelling companion
are also solely responsible for all costs incurred to and from the Departure Point
as the Grand Prize originates and terminates there. It is the sole responsibility
of the Grand Prize Winner and his/her travelling companion to obtain all necessary
travel documentation including passports and visas if applicable and to comply
with any customs and immigration requirements. Grand Prize Winner and his/her
travelling companion must be able to travel at the times and dates as determined
by the Contest Sponsors in their sole discretion and authority failing which the
Grand Prize will be forfeited. It is recommended that Grand Prize Winner and his/her
travelling companion obtain sufficient personal insurance prior to departure.
Travel must be completed by April 5th, 2004. Approximate retail value of the Grand
Prize is Two Thousand Seven Hundred Dollars Canadian ($2,700.00) based on a Toronto
departure. In the event that the Grand Prize winner has a permanent residence
within 100 KM of Edmonton, AB, the Grand Prize shall be deemed to not include
airfare or hotel accommodation. In these circumstances, no cash substitution will
be made.
3. SECONDARY PRIZES: There are fifty (50) Secondary Prizes each consisting
of one (1) copy of the Juno Awards 2004 CD. Approximate retail value of each Secondary
Prize is Twenty Dollars ($20.00 CDN)
4. Grand Prize(s) and Secondary Prize(s) (collectively "Prizes")
must be accepted as awarded. The Prizes may not be sold, transferred and are not
convertible to cash. No change in travel arrangements can be made once the booking
has been confirmed. The Grand Prize may not be used towards any type of frequent
flyer mileage, or other reward point accumulation program. Contest Sponsors reserve
the right to substitute the Prizes in whole or in part in the event that all or
any component of the Prizes are unavailable.
On or after March 26th, 2004 at approximately 12:00 noon. ET, a random draw will
take place at CTV Television Inc., 9 Channel Nine Court, Toronto, Ontario, from
all eligible on-line entries received during the Contest Period. The first entry
drawn will be eligible to win the Grand Prize. Then a random draw will take place
from all remaining eligible entries and the next fifty (50) entries drawn will
each be eligible to win a Secondary Prize. The odds of winning a Prize will depend
upon the total number of eligible entries received during the Contest Period.
The Contest Sponsors will attempt to contact each potential winner by telephone
within 1 business day after the draw. Proof of identification must be provided
upon request. In order to be declared a winner, each potential winner must first
correctly answer, unaided, a time limited mathematical skill testing question
and before being awarded the prize, (a) each potential winner will be required
to sign a full release and indemnity form stating that he/she has read, understood
and complied with these official contest rules, grants all consents required as
contemplated in these contest rules, authorizes the Contest Sponsors to broadcast,
publish and disseminate his/her name, sobriquet, address, photograph, likeness
and voice, in connection with any promotion or publicity, and/or for general news,
entertainment and information purposes at no additional compensation to the potential
winner, beyond the awarding of the Grand Prize, accepts the prize as offered and
releases the Contest Sponsors from any and all liability of any kind arising out
of the potential winner's participation in this contest and receipt and use of
the prize. Each potential winner's traveling companion will be required to sign
a full release and indemnity form stating that he/she releases the Contest Sponsors
from any and all liability of any kind arising out of his/her participation in,
and use of, the prize, and authorizes the Contest Sponsors to broadcast, publish
and disseminate his/her name, sobriquet, address, photograph, likeness and voice,
in connection with any promotion or publicity, and/or for general news, entertainment
and information purposes at no compensation to the traveling companionbeyond the
participation in the Grand Prize.
In the event that the potential winner is not eligible, cannot be contacted
by the Contest Sponsors within one (1) business day following the draw, does not
correctly answer the mathematical skill testing question within the time limit,
does not sign and return the release within the specified time, or the travelling
companion does not sign and return the release within the specified time, Contest
Sponsors shall have the right to disqualify that potential winner and to draw
an alternate potential winner and the Contest Sponsors shall be fully and completely
released and discharged from any liability or responsibility in this regard. The
provisions and procedures referred to above relating to selection and notification
of a potential winner shall be applied, with the necessary amendments, until a
qualified winner and traveling companion have been duly selected. If the travelling
companion of the Grand Prize Winner is under the age of majority, the release
must be signed by his/her parent or legal guardian on his/her behalf.
5. By entering this contest the entrants and participants automatically agree
to accept and abide by these contest rules. All decisions of the Contest Sponsors
with respect to any aspect of this contest, including without limitation the eligibility
of entries, are final and binding on all entrants in all matters as they relate
to this contest.
6. All entries become the property of Contest Sponsors who assume no responsibility
for garbled, lost, late, delayed, destroyed or misdirected mail, voice messages,
e-mail or any computer errors or malfunctions. No correspondence will be entered
into except with contestants requesting contest rules by mail or requesting a
list of prize winners (for which a self-addressed, postage paid envelope must
be included). Contest Sponsors do not assume any responsibility for incorrect
or inaccurate capture of entry information, technical malfunctions, human or technical
error, seeding or printing errors, lost, delayed or garbled data or transmissions,
omission, interruption, deletion, defect or failures of any telephone or computer
line or network, computer equipment, software or any combination thereof. Entry
materials/data that have been tampered with or altered are void. If for any reason,
in the opinion of the Contest Sponsors, in their sole discretion, the contest
is not capable of running as originally planned, or if the administration, security,
fairness, integrity or the proper conduct of the contest is corrupted or adversely
affected, including by reason of infection by computer virus, bugs, tampering,
unauthorized intervention, fraud, technical failures, or any other causes beyond
its control, the Contest Sponsors reserve their right to cancel, terminate, modify,
amend or suspend the contest and select a winner from previously received eligible
entries. The Contest Sponsors reserve their right in their sole discretion to
disqualify any individual it finds to be tampering with the entry process or the
operation of the contest or to be acting in violation of these rules or otherwise
in a disruptive manner. The Contest Sponsors reserve their right to modify the
rules of this contest without materially affecting the terms and conditions of
the contest. Contest Sponsors in their sole discretion, further reserve their
right to terminate the on-line portion of the contest and conduct the drawing
from all on-line entries previously received during the Contest Period. Any attempts
to deliberately damage any web site or to undermine the legitimate operation of
this contest is a violation of criminal and civil laws and should such an attempt
be made, the Contest Sponsors reserve their right to seek remedies and damages
to the fullest extent of the law. Contest Sponsors shall not be held responsible
for any errors or negligence that may arise or occur in connection with the contest
including any damage to an entrant's computer equipment, system, software or any
combination thereof, as a result of their participation in this contest or from
downloading any material from the contest web site.
7. The contest is subject to all applicable federal, provincial and municipal
laws and regulations. By entering this contest, each entrant consents to the collection,
use and distribution of his/her personal information (information that identifies
an entrant as an individual, such as home telephone number, age and home address)
by the Contest Sponsors for the purposes of implementing, administering and fulfilling
this contest. Contest Sponsors will not sell or transmit this information to third
parties except for the purposes of administering the Contest. Any inquiry concerning
the personal information held by the Contest Sponsors should be addressed to the

8. INSERT FOR QUEBEC CONTESTS ONLY: For Quebec residents, any litigation respecting
the conduct or organization of this contest may be submitted to the Regie des
alcools, des courses et des jeux du Quebec for a ruling. Any litigation respecting
the awarding of a prize may be submitted to the Regie only for the purpose of
helping the parties reach a settlement.
